In Motion Physical Therapy Clinics in Portsmouth Make the Change to Electronic Medical Records

Last week Bon Secours Maryview Medical Center and Bon Secours Health Center at Harbour View launched the health systems integrated clinical information system. ConnectCare enables health care providers to access a patient’s medical data in a single electronic health record (EHR), including medical history, allergies, test results and medications, in one secure and confidential electronic record. The patient’s care team has access to all treatment plans and safety alerts, ensuring continuity of care throughout a patient’s life.

Through ConnectCare, medical staff can readily access confidential patient information from any computer to review charts and analyze test results. Physicians, nurses and staff are presented with an immediate and comprehensive overview of a patient’s medical history, and multiple users can access the same patient record at the same time.

Marleena Debrough, MPT, a physical therapist at Bon Secours In Motion Physical Therapy in Portsmouth, demonstrates her ConnectCare proficiency on a work station on wheels, or WOW. Debrough says: “This is not a hard system to learn and I’m now working on ensuring that I am using ConnectCare while performing patient care!”

As the clinical teams at Bon Secours Maryview and Harbour View implement ConnectCare, there’s a team with a keen eye on quality measures – the Quality HUB. This team is comprised of members from care management, core measures teams, abstractors, physical therapy, risk management and other disciplines. Their role is to ensure that our clinical team continues to document that we are providing high quality care. Some of the elements that are reviewed include pain management, medication reconciliation, core measures, sepsis bundles, shift assessments, fall risk, etc.
